TIPS:

1. Save the form to your PC. When you click the button above, the voyage application form will be displayed in your browser. Do not start completing it!, but save it first to your computer.
2. Navigate to the folder on your computer where you saved this file.
3. Open the PDF file and start completing it. You need Adobe Acrobat Reader to do so. Visit Adobe website here and follow the prompt to download and install the free software on your PC.
4. Save the PDF file*  
5. Send it:
   1. Via email, as an attachment, to: office@sailleeuwin.com
   2. Via post, as a hard copy, to: PO BOX 1100 Fremantle WA 6959
